Delivery of Form 1095-C
In accordance with IRS regulations, the Kenai Peninsula Borough is no longer required to automatically mail the Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, to individuals.
Beginning with tax year 2025, these forms are now available by mail upon request in the event you need them to prepare your personal income tax return.

If you would like to receive a paper copy of Form 1095-C, you can request the form through one of the following methods:
